Olivia Forys, MD, came to Rush from the Mayo Clinic in Rochester, Minnesota. She is interested in depression and anxiety, hypertension and lipid management, and women's health. Forys is fluent in Polish.
Max J. Pitlosh, MD, received his medical degree from Michigan State University College of Human Medicine in East Lansing, Mich. He completed his residency in family medicine at Resurrection Medical Center in Chicago, where he served as chief resident.
Board-certified in family medicine, Pitlosh has a special interest in the treatment of cholesterol, hypertension, asthma and diabetes. He sees patients of all ages from infancy through old age.
Patricia Walsh, MD, has enjoyed a loyal following of patients of all ages, particularly women, for nearly three decades. She has a special interest in preventive heart disease and women's health issues including gynecologic care, menopause and osteoporosis.

Monday -- 8:30 a.m. to 7 p.m.
Tuesday -- 8:30 a.m. to 5 p.m.
Wednesday -- 8:30 a.m. to 5 p.m.
Thursday -- 9:30 am to 7 p.m.
Friday -- 8:30 a.m. to 2 p.m.
Saturday -- 8:30 a.m. to 1 p.m.
Our office offers complimentary parking at the valet parking lot located just north of our building. There is also metered street parking available on Sheffield Avenue and the surrounding side streets.
Our office is ideally located along the Brown El line, just 1-1/2 blocks north of the Diversey stop or 1-1/2 blocks south of the Wellington stop. You may also take the No. 76 Diversey Bus to the Sheffield Avenue stop and proceed north 1-1/2 blocks on Sheffield Avenue to our practice.
In addition to our normal clinic services, we offer urgent care/same-day services for patients 16 years of age or older through the Rush Walk-In Care Center. Services are available on a walk-in basis — with no appointment necessary — Friday (2 to 7 p.m.) and Saturday/Sunday (9 a.m. to 5 p.m.). Patients are seen by board-certified nurse practitioners, who are qualified to diagnose and treat acute conditions, including the following:
